Postwork Instructions

Postwork is due one week before the next live Learning Collaborative session. Postwork is intended to help your team develop and build on the skills you are learning during the live Learning Collaborative sessions. The QI coaches are available during their QI Coaching Office Hours to help with any questions regarding the postwork. The postwork attachment(s) are available at the bottom of this page.

Module 2: Aim Statements and Gap Analysis

Postwork due: July 10, 2024

Postwork Directions:

Postwork completed as a team:

1. Continue to meet regularly with your participating team members. 
2. Download the Aim Statement and Gap Analysis templates.
3. Discuss and complete the Aim Statement and Gap Analysis together. 
   1. Share team members' reflections and goals to brainstorm each section of the Aim Statement. Then, finalize the Aim Statement together. 
   2. Share team members' reflections on your health center gaps from the article you read to brainstorm the Gap Analysis. Then, complete the Gap Analysis together. 
4. Submit the Aim Statement on this page. (Only the team lead needs to submit)
5. Submit the Gap Analysis on this page. (Only the team lead needs to submit)

Once you have completed the postwork, select the Continue tab to submit your assignment.
