Postwork Instructions

Postwork is due one week before the next live Learning Collaborative session. Postwork is intended to help your team develop and build on the skills you are learning during the live Learning Collaborative sessions. The QI coaches are available during their QI Coaching Office Hours to help with any questions regarding the postwork. The postwork attachment(s) are available at the bottom of this page.

Module 3: Screening and Assessment

Postwork due: August 7, 2024

Postwork to be completed as a team:

1. Using the flowchart template on page 4 of the QI Essentials Toolkit: Flowchart, create a flowchart listing out all the steps in the process you are mapping. 
2. Be sure to include decision points, loops, handoffs between roles, etc...
3. Submit your flowchart (only team leads need to submit). 

Once you have completed the postwork, select the Continue tab to submit your assignment.
