Prework Instructions

Prework should be completed before each live learning collaborative session. The prework is specifically designed to help your team understand the material and to know what to expect for the live Learning Collaborative session. The prework attachment(s) are available at the bottom of this page.

Module 2: Aim Statements and Gap Analysis

Prework due date: June 19, 2024

Prework Directions:

Prework for each individual team member:

1. Download the Aim Statement Template. Review the Aim Statement template and reflect on each section. Write at least one goal you have for your health center to be shared during your next team meeting.
2. Download and Read "Obesity intervention evidence synthesis: Where are the gaps and which should we address first?" Reflect on the gaps within your health center. Did this article bring to light any gaps you didn't consider previously? These reflections will be shared during your next team meeting. 

Once you have completed the prework, select the Continue tab.
