Prostate Cancer Screening Learning Collaborative Application

Program Information

Funded by the Novartis U.S. Social Impact, the Weitzman Institute will offer a free learning collaborative on prostate cancer screening to organizations in Detroit, Houston, and Los Angeles. 

This eight-month learning collaborative is designed to provide participants with technical assistance and practical tools to design and implement site-specific improvements to their prostate cancer screening workflow. This learning collaborative will include two parts: 

  • Systems Integration & Quality Improvement (Part 1): Supports teams in identifying eligible patients for screening, understanding workflow and data gaps, map referral processes, and implementing system-level quality-improvement and change-management strategies that reduce missed screenings and delays. 
  • Addressing Social Barriers (Part 2): Guides teams in assessing and mitigating patient-level barriers such as transportation, work schedules and childcare, financial insecurity, and other social needs. 

Each part will include four sessions each, as well as four monthly Quality Improvement guidance and feedback sessions each. 

Applications are due on Friday July 10th. Following submission, applicants will be invited to a Zoom meeting to review the learning collaborative structure and expectations. Space is limited to 10–12 organizations, and each participating organization will receive a $12,50 0stipend!

Learning Collaborative Expectations

Below are key expectations for participating in this learning collaborative opportunity:

  • The organization should be located in Detroit, Houston, and Los Angeles. 
  • Each participating organization should have a team of about 3-5 interdisciplinary members that engage in 8-month learning collaborative.  
  • Ability to make changes to your electronic health record (EHR) to improve your processes for prostate cancer screening. 
  • Ensure adequate number of male patients over the age of 40 to implement changes 

Learning Objectives

Upon completion of this learning collaborative, participants should be able to do the following:

  • Use quality improvement tools and skills to identify areas for process improvement.
  • Design site-specific interventions to improve patient communication with at-risk patients about prostate cancer screening.
  • Evaluate patient-level barriers and develop strategies to improve care.
