
Session 1: Screening Tools (March 18, 2025)
Program Information
This learning collaborative supports school-based and community-based health centers in strengthening their screening practices for social determinants of health in children and adolescents. Through this initiative, participants will develop a sustainable screening approach to identify and address factors influencing a child’s well-being. The collaborative will provide guidance on the benefits of screening, practical implementation strategies, and best practices for care coordination to enhance health outcomes.

Learning Objectives
Upon completion of this activity, participants should be able to do the following:
- Identify evidence-based tools to detect social needs and strengths in children and adolescents.
- Strengthen school and community partnerships to address social influences of health needs.
- Apply best practices in social influences of health coordination and follow-up.
