Program Information
The SBHCC Care Coordinator training series provides essential education and skills for newly hired, non-credentialed care coordinators in school-based health settings, focusing on care coordination, social determinants of health, and collaboration with schools, students, and families. It equips coordinators and school-based health center staff with the tools needed to streamline care delivery, improve communication, and integrate services effectively.
The series includes 19 sessions, with 2 CEUs available for each session attended.

Learning Objectives
Upon completion of this activity, participants should be able to do the following:
- Identify Required Information and Documentation: List and explain the various types of information and documents required for intake prior to the initial care coordination visit, including legal consents and health records.
- Implement Effective Data Collection Techniques: Demonstrate proficiency in using MyCareCircle (MCC) for data collection, documentation, and management of care coordination cases
- Prepare for Follow-up and Ongoing Care Coordination: Prepare to schedule and manage follow-up appointments and ongoing care coordination needs based on initial screenings and assessments.
