Carolyn Read, MSN, MPH, MSW, RN, APRN, FNP-BC, ACHPN is a certified Palliative Care Nurse Practitioner and a dedicated Faculty Member for the Oak Street Health (OSH) New Graduate NP Fellowship in Geriatric Primary Care. Since joining the faculty in 2022, she has played numerous roles on the fellowship team, serving as an advisor, preceptor, didactic curriculum designer, and live learning instructor, while also leading recruitment efforts. Notably, she played a pivotal role in securing the fellowship’s recent accreditation with the Consortium for Advanced Practice Providers in November 2025. Alongside her Fellowship work, Carolyn provides home-based palliative care services to Oak Street patients throughout the Chicagoland area. Within her fellowship role, she especially loves advising, mentoring, and teaching the next generation of NPs as they launch their careers in specialized geriatric primary care.
