HCC, RAF, and Payment: Using Azara DRVS to Capture What You're Owed (9:30am-10:30am; October 9, 2026)

Session Information

Under PPS, better coding won't raise your visit rate — the payment case sits in the Medicare and Medicaid programs on top of it. This session covers where RAF turns into dollars: Medicare Advantage, where plan revenue is RAF-driven and reaches health centers through shared risk and quality arrangements; MSSP and ACO REACH, where a panel's coded acuity sets the benchmark you're measured against; and annual wellness visits and chronic care management, which are billable and double as the way to recapture chronic conditions, which must be re-documented every calendar year. On the Medicaid side, managed care rate setting and VBP arrangements use their own risk models, so complete coding still drives capitation and MCO incentive performance. We then show the DRVS workflow that covers both: point-of-care tools to empower providers and population-level reports to illuminate key areas for intervention. Coding and quality gaps largely overlap in the same patients, so a single workflow closes both.
