Program Information
This is a peer-learning opportunity for health centers interested in bringing SDOH screening into their practice. Learn about the benefits of SDOH screening and how to create and implement a sustainable screening practice to advance child & adolescent health.

Learning Objectives
By the end of this collaborative, participants will be able to:
- Conduct community needs assessments
- Determine screening tool selection criteria based on population and community needs
- Identify the key considerations for adapting clinic workflow to include SDOH screening
