Ho-Choong Chang, MD
Dr. “Ho” Chang is Associate Professor in Clinical Pediatrics, Yale University School of Medicine and serves as primary care provider and clinical instructor at the New Haven Primary Care Consortium in collaboration with Fair Haven Community Health Center. He previously served as the Chief of Pediatrics of Community Health Center, Inc. (CHCI), Connecticut state’s largest Federally Qualified Health Center providing care to over 35,000 medically underserved and marginalized children. Dr. Chang also served as the On Site Medical Director of Connecticut Pediatrics @ CHC in Hartford. In these roles, Dr. Chang provided oversight of the delivery of high quality, evidence-based health care to CHCI's pediatric population with an emphasis on integrated, team-based care.
Dr. Chang earned his undergraduate degree in psychobiology at Yale College, and received his medical degree from Johns Hopkins University. He completed his pediatrics residency at Seattle Children’s Hospital, University of Washington.
