NTTAP Comprehensive and Team-Based Care Learning Collaborative 2024-2025 (March 12th, 2025)
Program Information
The Comprehensive and Team-Based Care Learning Collaborative is a 8-month participatory learning experience offered by the National Training and Technical Assistance Partners (NTTAP) on Clinical Workforce Development, funded by the Health Resources and Services Administration (HRSA), and hosted by Community Health Center, Inc. (CHCI). The Learning Collaborative is designed to provide free training and technical assistance to support health centers beginning or restarting their move to high performance team-based comprehensive primary care with knowledge about the basic principles and best practices of care and the strategies to plan for implementation. Each participating health center will identify a team, organization leadership representation, and a designated coach to lead the team through the work of the collaborative.
Learning sessions will meet monthly on Wednesdays of every month from 1:00-2:30pm ET / 10:00-11:30am PT.

Learning Objectives
Upon completion of this activity, participants should be able to do the following:
- Use assessments of their current team-based care model to identify areas for process improvement and role optimization
- Use quality improvement concepts and skills with coaching support to systematically achieve one or more specific aims.
- Contribute to the learning among participating practice teams by engaging in Learning Collaborative activities.
