This ECHO program connects healthcare providers and care team members to a community of peers and subject-matter experts from Ohio University to address patients’ complex health needs related to diabetes. At each session, subject-matter experts from Ohio University will provide advice and guidance from a multidisciplinary team approach to address patients’ complex health needs related to diabetes to screen, assess, treat, and manage patients with all types of diabetes. Weitzman ECHO Diabetes Across the Lifespan is a no-cost, 12-month, 24-session program.
