Program Information

Join Community Health Center, Inc. (CHCI) and Health Outreach Partners (HOP) for a free two-part webinar series sponsored by the National Health Center Training and Technical Assistance Partners (NTTAP). This series is designed to provide participants with foundational knowledge on how to utilize team-based care to address mental health disparities during and after pregnancy.

In this first webinar, expert panelists will discuss an advanced model of team-based care that can be used for addressing maternal health disparities. The panelists will dive into the application of team-based care strategies and the roles within the interdisciplinary team including the primary care provider, certified nurse midwife, registered nurse, behavioral health, and more.
