The Advancing Team-Based Care Learning Collaborative is a 4-month participatory learning experience offered by the National Health Center Training and Technical Assistance Partners (NTTAP), funded by the Health Resources and Services Administration, and hosted by Community Health Center, Inc. (CHCI) in Middletown, CT. The collaborative is designed to provide Federally Qualified Health Centers (FQHCs) transformational strategies and coaching support to help primary care practices implement and advanced models of team-based care.

Learning Objectives

Upon completion of this activity, participants should be able to do the following:

  • Use assessments of their current team-based care model to identify areas for process improvement and role optimization
  • Use quality improvement concepts and skills with coaching support to systematically achieve one or more specific aims
  • Contribute to the learning among participating practice teams by engaging in Learning Collaborative activities

Faculty

Deb Ward, RN is a Senior Quality Improvement Manager at Community Health Center, Inc.'s Weitzman Institute. She has extensive managerial experience in both acute and primary care. Before joining the Quality Improvement Department, Deb was the Nurse Manager at CHC's Meriden site, and has worked at the health center for ten years. She is the current Chair of the Patient Education Committee. Ms. Ward has held primary responsibility for coordination of grants that focused on improved Hypertension and Diabetes control. She has completed Clinical Microsystems* training at the Dartmouth Institute, receiving her Coaching Credentialing Certificate and currently coaches several teams. Ms. Ward is also CHC's lead coach, responsible for providing ongoing support for all of CHC's coaches and the continuing spread of Microsystem teams at CHC.

Kathleen Thies, PhD, RN has worked with Community Health Center, Inc. since January 2014 on several projects. She directed the implementation and evaluation of the roll out of nurse-driven care coordination across all 12 clinical sites in the agency. As part of that initiative, she helped to develop the curriculum for nursing staff on care coordination and the health information technologies that lend decision support to the nurses, and is responsible for evaluation of the care coordination project in partnership with the Yale School of Public Health. She is currently working on the Project ECHO for LGBT, in which CHC is a partner with the National Association of Community Health Centers, the CDC, and Fenway Health in Boston. Dr. Thies has also done grant writing for CHC.
