
Annual Training Institute: Healthcare for the Homeless Track (May 20, 2026)
Program Information
Each year, thousands of individuals across Connecticut experience homelessness, often facing complex and intersecting physical, behavioral, and social health challenges. Addressing these needs requires coordinated, integrated approaches across healthcare and housing systems.
The Healthcare for the Homeless Symposium has long served as a platform to elevate best practices, amplify lived experience, and foster collaboration. The Healthcare for the Homeless (HCH) Symposium is proud to partner with the Connecticut Coalition to End Homelessness (CCEH) as part of the 2026 Annual Training Institute (ATI). This collaboration brings together healthcare providers, homeless service organizations, policymakers, and community partners in a unified, statewide convening focused on strengthening our collective response to homelessness.
As a dedicated track within ATI, the HCH Symposium elevates critical conversations around healthcare delivery for individuals experiencing homelessness. Through engaging sessions, expert-led discussions, and cross-sector collaboration, the HCH track highlights innovative, person-centered, and trauma-informed approaches that improve health outcomes and advance health equity across Connecticut.
This year’s theme, “Together We Learn, Together We Lead: Creating a Culture of United Response,” reflects the power of partnership—recognizing that meaningful progress happens when healthcare and housing systems work in alignment.

Learning Objectives
Upon completion of this activity, participants should be able to do the following:
- Identify at least three key clinical, behavioral health, or social determinants impacting individuals experiencing homelessness and describe their effect on care delivery.
- Apply at least two evidence-based strategies to improve care coordination and patient engagement within their practice setting.
- Demonstrate the ability to integrate interdisciplinary approaches into workflows to address complex patient needs.
- Evaluate current practices and identify one area for improvement to enhance quality, safety, or health outcomes.
- Implement at least one actionable change in practice that supports equitable, patient-centered care for underserved populations.
