ForwardCHC/HOP Webinar Series on Advancing Team-Based Care to Address Maternal Health Disparities (May 2024)
Program Information
Join Community Health Center, Inc. (CHCI) and Health Outreach Partners (HOP) for a free two-part webinar series sponsored by the National Health Center Training and Technical Assistance Partners (NTTAP). This series is designed to provide participants with foundational knowledge on how to utilize team-based care to address maternal health disparities during and after pregnancy.
In the first webinar, expert panelists will discuss an advanced model of team-based care that can be used for addressing maternal health disparities. The panelists will dive into the application of team-based care strategies and the roles within the interdisciplinary team including the primary care provider, certified nurse midwife, registered nurse, behavioral health, and more.
The second webinar will be on the topic of maternal mental health. Expert panelists from Alameda County in California will discuss innovative programs that incorporate mental health for new and expecting mothers, as well as hear about challenges and successes when implementing mental health screening tools in health centers. Participants will leave with a deeper understanding of the importance of screening for mental health among new and expecting mothers, as well as incorporating whole-child, whole-family approaches to public health interventions and programs to tackle maternal health disparities.
