Module 3: Data-Driven Navigation – Proving Impact Through Smart Metrics (September 10, 2025)

Program Information

This virtual, peer-led CoP will equip health centers with forward-thinking, results-driven strategies to build, optimize, and sustain patient navigation programs that significantly improve access to timely cancer screening and diagnostic follow-up. Through interactive sessions, participants will explore innovative tools and models, tackle real-world barriers, and adopt proven practices to elevate the impact of navigation in underserved communities. Participants will leave each session with concrete ideas and implementation tools they can apply immediately.
