Nutrition's Role in Patient-Centered Care: Practical Approaches for Primary Care

Program Information

This webinar will provide primary care teams with practical, evidence-based strategies for incorporating nutrition into the prevention and management of chronic disease. Participants will review the fundamentals of nutrition, including the role of macronutrients, micronutrients, physical activity, and dietary patterns in supporting overall health, while exploring the relationship between nutrition and common chronic conditions. Participants will also be introduced to motivational interviewing strategies and other evidence-based tools and resources that can support sustainable health behavior change and improve patient outcomes.

Drawing on insights from our Weitzman ECHO Fusion series on nutrition and chronic disease and led by the same multidisciplinary faculty, this webinar brings key best practices and lessons learned to a broader audience. No prior participation in the ECHO series is required.

  • Explain the relationship between nutrition and common chronic conditions and identify dietary strategies that support disease prevention and management.
  • Apply a patient-centered approach to assessing nutrition-related needs and prioritizing realistic, individualized behavior change goals.
  • Determine tools and resources to assist in implementing evidence-based dietary counseling techniques to support patients with sustainable behavior change, including motivational interviewing.
