SDOH Screening Start-Up Learning Collaborative
Program Information
The School-Based Health Alliance is offering a peer-learning opportunity for health centers interested in bringing SDOH screening into their practice. Learn about the benefits of SDOH screening and how to create and implement a sustainable screening practice to advance child & adolescent health.
Target Audience

Learning Objectives
By the end of this collaborative, participants will be able to:
- Conduct community needs assessments
- Determine screening tool selection criteria based on population and community needs
- Identify the key considerations for adapting clinic workflow to include SDOH screening

Through Joint Accreditation, Community Health Center, Inc./Weitzman Institute is able to provide American Nurses Credentialing Center (ANCC) nursing credit to nurses, Accreditation Council for Pharmacy Education (ACPE) credit to pharmacists, AMA PRA Category 1 Credit™ to physicians at its activities via Accreditation Council for Continuing Medical Education (ACCME), American Academy of PAs (AAPA) credit to physician assistants, American Psychological Association (APA) credit to psychologists and Association of Social Work Boards (ASWB) credit to social workers at its activities.
