THNP Interprofessional Healthcare Navigation Learning Series™ (September 15, 2026)

Program Information

The THNP Interprofessional Healthcare Navigation Learning Series™ (2026-2027) is an interactive continuing education program designed for Connecticut school-based health center professionals, including medical, dental, behavioral health, nursing, and care coordination staff. Through a series of virtual sessions, participants will strengthen their ability to support children, adolescents, and families in navigating complex healthcare systems, promoting self-advocacy, accessing specialty and behavioral healthcare, managing transitions from pediatric to adult care, understanding pharmacy resources, and addressing financial and technology-related aspects of healthcare. 

This evidence-informed educational series provides practical, immediately applicable strategies to improve care coordination, interdisciplinary collaboration, patient engagement, and equitable access to services. Participants will gain tools and resources that can be integrated into daily school-based health center practice to reduce barriers to care, improve healthcare navigation, and ultimately enhance health outcomes for Connecticut's students and families.

This activity meets the third Tuesday of every month from 12-1:30pm ET.
