Weitzman ECHO Diabetes Across the Lifespan - Aging Module (February 21, 2023)

Program Information

The Weitzman ECHO Diabetes Across the Lifespan program connects primary care medical providers and clinicians to a community of peers and subject-matter experts from Ohio University to address patients’ complex health needs related to diabetes. At each session, subject-matter experts from Ohio University provided guidance from a multidisciplinary team approach to address patients’ complex health needs related to diabetes to screen, assess, treat, and manage patients with all types of diabetes. The Weitzman ECHO Diabetes Across the Lifespan is a no-cost, 12-month, 24-session program beginning October 18th, 2022 and will meet for 1-hour, 1st and 3rd Tuesdays from 12-1 pm ET.

This ECHO series is organized into the following modules to explore diabetes throughout different stages of life and in specific populations:

  1. Aging Adult Module
  2. Pregnant and Postpartum Module
  3. Adolescent Module
  4. Pediatric Module

This ECHO series is now enrolling primary care medical providers and clinicians from Ohio and surrounding states. All providers working in primary care settings (especially FQHCs) are encouraged to join and participate. CME, CNE, CPE, and CEU credits will be provided for physicians, nurse practitioners, physician assistants, nurses, and registered dietitians, among others, by Community Health Center, Inc., an accredited provider through Joint Accreditation for Interprofessional Continuing Education.

Learning Objectives

Related to diabetes in the aging population and primary care, by the end of this activity, participants will be able to:

  1. Apply evidence-based practices to effectively treat all types of diabetes mellitus as people age
  2. Employ best practices in screening and assessment to determine appropriate levels of care for common medical and behavioral health conditions
  3. Treat and manage complications and co-morbidities more effectively
  4. Better facilitate and improve medication management for patients with diabetes
  5. Apply self-management approaches with patients to empower them to manage diabetes and improve physical activity, sleep, and healthy eating
  6. Connect and help patients navigate services to address barriers to care and social determinants of health to improve health outcomes
