Program Information
The Pennsylvania Association of Community Health Centers (PACHC) 2025 Annual Conference and Clinical Summit is where Pennsylvania’s vibrant “community of Community Health Centers” come together for three days of connection, learning, and inspiration. This year’s event will bring together health center leaders, providers, and staff from across the state for: professional development sessions, timely and impactful conversations, innovative clinical insights, networking opportunities and an inspiring keynote speaker.

Weitzman Institute Disclosure Statement

- Select the Register button above to register for all conference sessions, regardless of your attendance.
- Then, in the table above, select the links for each breakout round you attended.
- Within each activity, select the Continue tab above to complete the session evaluation. You will need to complete a session evaluation for each breakout round you would like to claim credit for.
- After completing session evaluations and claiming credit for each session you attended, you can download an overall conference certificate appropriate to your credentials.

You must claim credit and download your certificate by the activity expiration date. For compliance reasons, the Weitzman Institute cannot modify this deadline. If you need any technical support, submit a ticket to our Support Center.
