Weitzman ECHO Diabetes and Weight Management 2025
Program Information
This ECHO program meets on the 2nd and 4th Tuesdays of each month from 2-3pm ET / 11am-12pm PT starting on January 28th.
Weitzman ECHO Diabetes and Weight Management connects primary care providers, behavioral health providers, and care team members with a multidisciplinary team of experts and specialists in type 2 diabetes and weight management. At each session, subject-matter experts provide guidance from a multidisciplinary team approach to address patients' complex health needs related to type 2 diabetes with or without obesity to better assess, treat, and care for patients.
This ECHO is a no-cost 14-session series that uses twice-monthly videoconferencing sessions to improve quality of care and clinical outcomes for underserved patients. Providers and care team members working in primary care settings (especially FQHCs and other safety net settings) are encouraged to join. CME, CNE, CPE, and CEU credits will be provided for physicians, nurse practitioners, physician assistants, nurses, registered dietitians, and pharmacists, among others, by Moses/Weitzman Health System Inc., an accredited provider through Joint Accreditation for Interprofessional Continuing Education.

Learning Objectives
After this learning series, participants will be able to:
- Facilitate conversations with people with type 2 diabetes with or without obesity using language that promotes respect, reduces bias, and develops trust.
- Analyze screening tool results to address factors such as social determinants of health and comorbidities.
- Individualize health-related behavior modifications to effectively manage type 2 diabetes with or without obesity in collaboration with people seeking care.
- Apply evidence-based strategies, including pharmacological and surgical interventions, based on available treatment options.
- Adjust treatment approaches for people with type 2 diabetes with or without obesity experiencing comorbidities and complications.

FAQs
What is Project ECHO®? Project ECHO® was developed by Sanjeev Arora, MD, at the University of New Mexico Health Sciences Center and is a collaborative model of medical education and care management that helps improve clinicians' knowledge and skills to improve patient care. ECHOs are designed to be longer-term continuing education programs to create a virtual community for providers and care team members to connect and learn from leading experts and receive guidance from peers and experts. This ECHO learning community provides mentorship and peer support which make this model unique and provides long-lasting impact beyond typical e-learning styles. Weitzman ECHO clinics are specifically designed to meet the needs of safety net primary care providers and their communities like providers practicing in federally qualified health centers (FQHCs).
How is each ECHO session structured? Each ECHO session consists of a didactic presentation by the ECHO's expert faculty and a real patient case discussion presented by participating providers. During each ECHO session, participants learn skills and receive clinical pearls from the expert faculty didactic presentation and the case discussion and recommendations.
How much does it cost to participate? Enrollment is now open at no cost for the next curriculum year.
Who should participate? Primary care medical providers, behavioral health providers, nurses, dietitians, pharmacists, or other members of the care team are welcome to join and register.
When do sessions occur? This ECHO will occur twice monthly for one hour. Our kickoff session will occur on January 28th, and sessions will occur every 2nd and 4th Tuesday from 2pm-3pm ET / 11am-12pm PT.
What level of participation is expected? Diabetes and Weight Management ECHO consists of 14 sessions. ECHO participants are expected to:
- Attend at least 75% of sessions
- Complete pre- and post-surveys
- Submit at least one case for recommendations
- Share webcams, when available
- Engage during peer case discussion
How do I join the live sessions? Do I need any special equipment? Weitzman ECHO sessions are held virtually using the Weitzman Education Platform and Zoom Video Conferencing. How do I claim credit and download my CE/CME certificate? After the live session is completed, the session evaluation will be available right on the platform. Once you complete the session evaluation, you will be able to claim your credits and download your certificate.

Faculty
Elizabeth Beverly, PhD is a Professor in the Department of Primary Care at the Ohio University Heritage College of Osteopathic Medicine. She is the recipient of the Osteopathic Heritage Foundation Ralph S. Licklider, D.O. Endowed Professorship in Behavioral Diabetes. Dr. Beverly graduated from The Pennsylvania State University with a Doctor of Philosophy degree in Biobehavioral Health in 2008. She completed a five-year postdoctoral fellowship in Behavioral Diabetes at Harvard Medical School with the Joslin Diabetes Center in 2013. Dr. Beverly’s research in diabetes focuses on understanding the linkages among psychosocial concerns, self-care, and health outcomes. She utilizes a mixed-methods approach to explore the intersection of culture, social drivers of health, and diabetes management within rural Appalachian communities in Ohio.
Mandeep Brar, MD is a board certified Endocrinologist with extensive experience in the management of complex endocrine disorders. After finishing her residency in Los Angeles, California she pursued her fellowship in endocrinology, diabetes and metabolism at the University of Colorado, Denver under the mentorship of renowned faculty such as Dr. Chester E. Ridgeway and Dr. Robert Eckel. After spending time on bench research in Colorado, Dr. Brar shifted her focus to develop clinical protocols for the management of diabetes and served as medical director at Virginia Mason Franciscan Health diabetes center in the greater Seattle area. Dr. Brar moved to Phoenix, Arizona in 2014 to take over Tenet Healths' Abrazo Diabetes Care Centers. During the past seven years, Dr. Brar has focused on population health interventions for management of diabetes and training the next generation of providers.
Kara Lewis, PharmD, AAHIVP is a clinical pharmacist leader at Community Health Center, Inc. Dr. Lewis has experienced working directly with patients in past roles including CVS Pharmacy and a Walgreens Pharmacy embedded within Community Health Center, Inc. Dr. Lewis provides significant support to providers on Project ECHO Key Populations including discussions on medication adherence and recognizing drug-drug interactions. Dr. Lewis has served on the Project ECHO Key Populations faculty since 2017.
Joy Mockbee, MD, MPH is a family physician and medical director at El Rio Health in Tucson, Arizona, where she has been caring for her patient panel for 25 years and continues to be energized by the innovation and mission of community health centers. She attended medical school at the University of Arizona and completed a Master of Public Health at the Harvard School of Public Health, followed by a Family Medicine residency at Ventura County Medical Center. She then returned home to Tucson, Arizona and has been a part of the community health center movement ever since. Responding to a request by El Rio Health staff and patients, Dr. Mockbee became interested in medical weight management and pursued board certification through the American Board of Obesity Medicine in 2015. Since that time, she has collaborated with a multidisciplinary team to develop El Rio's Weight Management programs for adults and children and to promote the practice of medical weight management by primary care clinicians throughout the health system.
Natalie Nyapete, MS, RDN has been practicing as a registered dietitian since 2016 after obtaining her Masters of Science in Nutrition and Dietetics from Saint Louis University. She has experience working with medically and socially complex patients bolstered by her time working at a non-profit serving food-insecure individuals with HIV/AIDS or cancer in St. Louis, MO. Natalie also worked in employee wellness and built programs for the staff and faculty of Washington University in St. Louis before moving to Tucson, AZ and joining the team at El Rio Community Health. At El Rio, Natalie has furthered her education and experience particularly in the areas of eating disorders and OBGYN care. Natalie and the team of dietitians at El Rio practice weight neutral care to provide patient-centered, evidence-based care that meets patients where they are and promotes positive behavior change free from stigma.
